SDM College of Physiotherapy

The establishment of SDM College of Physiotherapy, in the year 1996, at Dharwad (Karnataka), is a fine example of the foresight of our Chancellor, Poojya Shri D. Veerendra Heggade. The College is situated in a picturesque surrounding between Dharwad and Hubballi- Dharwad are well connected by road, railway, and airways to other major parts of the country such as Goa, Maharashtra, Andhra Pradesh, Tamil Nadu, Delhi, Gujarat, etc. The college is a constituent unit of Shri Dharmasthala Manjunatheshwara University and is recognized by the Govt. of Karnataka. The college has well qualified & experienced staff with different specialties to provide knowledge and skills to the students. The campus is a sprawling 70 acres campus with all amenities inside the campus. The institute is equipped with spacious classrooms, which are well furnished and equipped with audio visual aids. The college has a library which is up-to-date with books, journals and catalogues along with internet facilities. Both hostel and mess facilities are available for students on the campus. Facilities like a well-equipped gym, tennis court, swimming pool, indoor court and play ground for sports activities are available for student utilization. The campus also houses a bank, ATM's, cafeteria, prayer hall, stores, auditorium etc. A 1200 capacity auditorium is also located in the campus for annual cultural activities, conferences, workshops and meets.

The Government of Karnataka in its GO HFW 84 MMC 2020 Bangalore dated:31-08-2019, has authorized the Directorate of Medical Education for selection of candidates through centralized online counseling by Karnataka Examination Authority for admission to B. Sc Nursing, BPT, B.P.O, and B.Sc. Allied Health Sciences courses for the seats in Government colleges and Government seats in private colleges.

Application Fee: 

General and OBC candidates Rs. 800/- 
SC, ST, Category – I and PWD candidates Rs.400/-

Academic Eligibility:

For admission to Ist year B.P.T Course: Applicant should have passed II-PUC / 12th Standard (10+2) or equivalentexamination individually in the subjects of Physics, Chemistry, Biology andEnglish and must have obtained not less than 40% marks taken together inPhysics, Chemistry and Biology (35% in case of SC/ST & OBC applicants).

Reservation of Seats:-

Seats will be reserved for the following categories to the extent it is specified by the Government.

1. The SC / ST/Category-I / 2A, 2B, 3A, and 3B and other eligibility criteria are applicable to Karnataka candidates who are eligible to claim eligibility for Government seats under clauses (a), (b), (f), (h), (j), (k) and the same is not applicable to eligibility clauses (c), (d), (e), (g), (i) to claim Government Seats and Government quota seats.
2. Reservation of seats in certain categories such as Rural, Kannada, etc and in Special Categories such as NCC, Sports, etc is applicable only to Karnataka candidates who are eligible for Government seats and Government Quota seat

Certificates in original along with one set of attested copy to be produced at the time of Document Verification:

• Final printout of the online application form.
• Proof for having paid the application fees.
• S.S.L.C marks card (for proof of Date of Birth).
• Marks Card of II-PUC or equivalent examination.
• Study Certificate countersigned by the concerned BEO/DDPI NOTE: CBSE, CISCE candidates should produce the study certificate obtained from the Principal of their respective schools. Further, countersignature of BEO or DDPI is not required for them

If applicable the following documents should be produced,

• Kannada Medium Study Certificate countersigned by the concerned BEO, for the candidates claiming Kannada Medium reservation benefits.
• Rural Study Certificate countersigned by the concerned BEO and a verification certificate (only for GM candidates).
• Caste Certificate / Caste Income Certificate / Income Certificate obtained from the concerned Tahasildhar, for the candidates claiming Reservation benefits. (Caste and Income Certificate in Form ‘D’ in case of Scheduled Castes and Scheduled Tribes and in Form ‘E’ in case of Category-I and a Caste Income Certificate in Form ‘F’ for other backward classes like 2A, 2B, 3A, and 3B, issued by the jurisdictional Tahasildhar)Domicile Certificate for 10 years in Karnataka (Issued by the Tahashildar of Karnataka) or Study Certificate of seven years in Karnataka.
• Hyderabad-Karnataka Region Reservation (Article 371 (j):) Candidates claiming must submit the certificate in Annexure – A (eligibility certificate – rule 3(3)) issued by the concerned Assistant Commissioner during document verification.
• Parent’s study certificate / Parent’s Hometown certificate / Parent’s Domicile certificate / Parent’s Employment certificate / Parent’s Cumulative Record / Parent’s Marks Card, etc., for the candidates claiming eligibility for Government seats based on the domicile/study/employment of the parent. xi. For Horanadu, Gadinadu Kannadiga candidates, and Anglo Indian Candidates: Affidavit Duly sworn in for the candidates who claim eligibility under Horanadu and Gadinadu Kannadiga clauses & Anglo Indian clauses.
• Parent’s study certificate / Parent’s Hometown certificate / Parent’s Domicile certificate / Parent’s Employment certificate / Parent’s Cumulative Record / Parent’s Marks Card, etc., for the candidates claiming eligibility for Government seats based on the domicile/study/employment of the parent.
• For Horanadu, Gadinadu Kannadiga candidates, and Anglo Indian Candidates: Affidavit is Duly sworn in for the candidates who claim eligibility under Horanadu and Gadinadu Kannadiga clauses & Anglo Indian clauses.
• The candidate shall be eligible to claim caste, category, and income benefit only if he has claimed the same in the online application form. Candidates belonging to SC, ST, and Category-I claiming seats under reserved category shall also produce an Income certificate issued by the jurisdictional Tahsildar for verification if fee exemption is to be claimed.
• Relevant Certificates for those who are claiming an exemption for seven years of study in Karnataka.
• Physical Fitness Certificate

Guidelines to fill the online Application form:

Since the Applicant should upload their photographs in online application entry, the applicant should scan their passport size photograph (Snapped with Name & Date on the photograph) in .jpg format and make it available for uploading with the following specifications.

Width = 1.3 inches, Height = 1.6 inches, Resolution = 200 X 200 dpi and the file size should be less than 100kb. 

Select the Course – “B.Sc Nursing/ BPT / BPO / B.Sc Allied Science” 

1. Name of the Candidate: Fill the name in BLOCK LETTERS as per the 10th standard Marks Card.

 2. Mother’s Name: Mother’s name in full should be filled. 

3. Father’s Name: Father’s Name should correspond to the form and spelling in the 10th standard Marks Card. 

4. Date of Birth: The Date of Birth should correspond to the entry in the the10th Marks Card in dd/mm/yyyy.

 5. Place of Birth: Select the District in which you were born. 

6. Gender: Select from the drop-down list Female/Male. 

7. Nationality: Enter the Nationality. 

8. Qualifying Examination: PUC (10+2) or Equivalent Examination

 9. Marks obtained in II PUC (Physics, Chemistry, Biology, Mathematics) 

10. Domicile of Karnataka: Select from the drop-down list Yes/No.

 11. Category: Select the respective category and enclose the certificate obtained from the Tahashildar of Karnataka

How to apply:

The sharing of seats in respect of Government seats and Management seats in Private Colleges:-

In Government Order No. HFW 211 RGU 2016 dated:28/09/2016, the percentage of seat-sharing in the Private Nursing and Physiotherapy colleges in the State has been published with respect to Graduate Courses for the academic year 2018-2019, in the ratio of 20:80 as 20% for the Government Quota and 80% of Management Quota respectively. 

2023-24 Admission to Master of Physiotherapy in Government colleges and Government seats in Private Aided / un-aided Minority & non minority Colleges in Karnataka. 

Eligibility: (Master of Physiotherapy)

(i) Candidate should be a citizen of India and domicile of Karnataka. 
(ii) Candidate who has passed or appearing the examination/internship B.Sc. (PT) or BPT degree from this University or any other University located in Karnataka state recognized as equivalent by RGUHS with not less than 50% of marks in aggregate are eligible. And such applicants should produce all documents including internship on or before counseling date.
(iii) Candidate should not have attained the age of 45years on the last date fixed for submission of application to PG Degree Entrance Test. 
(iv) Candidates who have completed or are completing Internship on or before date of counseling. 

Registration Fee: 

General and OBC candidates Rs. 1500/- SC, ST, 
Category - I & PH candidates Rs.800/- 

Reservation in favour of certain categories: 

Orders issued by Government of Karnataka from time to time for reservation in seats for constitutionally permissible classes of Karnataka candidates such as SC and ST and Category I, IIA, IIB, IIIA and IIIB, are applicable. Failure to produce the latest income and category certificate issued by Tahashildar of Karnataka will result in forfeiture of his/her claim for reservation and he/she will be considered as a candidate belonging to General Category only. Requests for change of category will not be entertained after submission of the application. 

The candidates claiming Reservation under Hyderabad Karnataka Region. 

The candidates claiming Reservation under Hyderabad Karnataka Region as local persons should produce the relevant certificate issued by the Jurisdictional Assistant Commissioner of the Revenue Sub-Division for claiming reservation under Article 371J (Hyderabad - Karnataka Region). Non production of the certificate during Document Verification will result to rejection of his claim. 

Certificates to be produced at the time of Document Verification:

i. S.S.L.C. Marks Card (For age proof and parent’s name). 
ii. Birth Certificate/Cumulative Record/any other certificate for proof of place of birth. 
iii. B.Sc (Nursing)/PC B.Sc (Nursing)/BPT Marks Card / B.Pharm Marks Card of all Phases/Years 
iv. Degree Certificate issued by the University 
v. Attempt Certificate issued by the Principal of concerned college 
vi. Compulsory Rotatory Internship/Industrial Training Completion Certificate or Certificate indicating the likely date of completion, issued by the Principal of the College. 
vii. Council/Board Registration Certificate. B.Sc (Nursing), PC B.Sc Nursing/ BPT / B.Pharm. 
viii. Professional Experience Certificate in case of Nursing 
ix. Domicile Certificate issued by the Tahashildar of Karnataka or Study Certificate from 1st to 10th Standard. 
x. Valid Income & Category Certificate obtained from the concerned Tahashildar of Karnataka, if reservation is claimed. 
xi. Certificate under article 371-J for claiming HK region reservation issued by the competent authority. Certificate under article 371-J for claiming HK region reservation issued by the competent authority. 
xii. As per G.O No. DPAR 20 SBC 82, dated 28th December,1983 in case of Married women category certificate in the name of candidate name along with Husband’s name. 
xiii. Any other Certificates required.
